The blank health surveys and their rationale in this folder were composed by Medical Committee for Human Rights member Margo Lukas, in hopes of collecting better information on the health, nutrition, and family planning needs of black Mississippi residents AND for passing on this accumulated knowledge to incoming medical volunteers. There's also a survey about segregation of hospital services and facilities.
